CASES OF ADULT STILL’S DISEASE

Still's disease is still an unexplained disorder that is characterized by high spiking fevers, transient salmon-colored rash and/or arthritis.
Materials and Metods:
We examined 12 cases retrospectively diagnosed as Still’s Disease during the last 5 years.
Findings:
Mean age of the patients was 20,9 ± 4,75 years at the time of diagnosis. Fever, arthritis and arthralgia were present in all patients. In 58.3 % of patients rash, splenomegaly and lymphadenopathy; in 41.6 % of patients hepatomegaly was detected. Erythrocyte sedimentation rate, serum ferritin level and white blood cell counts were high in all patients. Chronic disease anemia, negative antinuclear antibodies (ANA) and rheumatoid factor (RF) results were also found in all patients. Elevated liver enzymes thrombocytosis, high level of serum fibrinogen and CRP were found to be 33.3 %, 75 %, 58.3 % and 83.3 % respectively. Eleven of the patients are still under our control but one died due to acute hepatic failure. Remission was present in 9 patients. Two of them have had occasional flare of the arthritis, arthralgia and fever. Normochromic normocytic anemia and the infections were the most frequent complications of the disease. One case was treated with erythropoietin.
Discussion and Conclusion:
Still’s disease is a disease that some cases can not be diagnosed due to some diagnostic difficulties. There are no standardized treatments and management protocol for the disease. Therefore, large case series are still needed to standardize diagnosis and treatment of this mysterious disease. Our results may help to build these kinds of standardizations even with its small numbers.
